NHS Trusts require digital transformation of their hospital fault reporting processes and infrastructure to address safety concerns and operational inefficiencies.

Background

The NHS loses £900m annually due to inefficient fault reporting. Our solution needed to transform a fragmented, time-consuming process into a seamless digital experience that works within the unique constraints of a clinical environment.

Clinical Environment Research

Shadowed staff (remotely due to Covid) across 4 hospitals to understand workflow patterns. Mapped existing fault reporting journeys and identified critical pain points. Conducted surveys and contextual inquiries with 41 clinical staff. Triangulated and synthesised insights to inform design decisions.

UX Design & Prototyping

Ensured strict adherence to Government Digital Service (GDS) standards, which was crucial for NHS digital transformation. Created rapid prototypes optimised for one-handed mobile use. Tested prototypes to validate usability.

Alpha Stage Development

Designed for existing NHS infrastructure but we began with a mobile-first approach based on insights. Technology integration paths were handled by architects.

Testing & Refinement

Conducted usability testing across personas. Gathered feedback through staff focus groups. Measured time-to-report.
